As a Case Manager within our Sydney West Family Based Care team in Baulkham Hills, you will work closely with Children, Young People, Carers & Families to develop and implement personalised case plans which work actively towards and promotes the permanency goals of the Children and Young People in our care.
To be successful in this role, you will have experience working with Children & Young People in out-of-home care and previous experience managing complex caseloads. Regular travel to the locations of the Children, Young People, Families and Carers is a requirement of this role, and a company vehicle provided for this.
Successful candidates will be required to clear probity checks including National Criminal History Record Check and NSW Working with Children Check.
