We deliver services in the areas of disability, out-of-home care, aged care, mental health and support for refugee and asylum seekers. At Life Without Barriers we all share the responsibility for child safety.

Partners in Recovery (PIR) is an innovative program directed at supporting people in the community with severe and persistent mental illness and complex needs. PIR will assist and coordinate multiple services and support systems to work in a more collaborative, integrated way so that people with severe and persistent mental illness and complex needs are better supported to access the right service at the right time in their journey to recovery.

About the Role

LWB has an exciting opportunity for an Intake Officer to join our team in Adelaide on a full-time basis until 30 June 2017 with possibility of extension due to funding.

The Intake Officer PIR works at both a client and systems level to ensure effective and appropriate referrals are received for PIR client group.

Key responsibilities include:

  • Receiving and processing PIR referrals, assessing eligibility, allocating to a host organisation for care coordination and entering client information needed for the client information management system.
  • Actively assisting ineligible clients access services as needed.
  • Assisting in identifying service system gaps and barriers relevant to the target group.
  • Liaising with GPs, other health professionals, agencies and organisations involved in the program as required.
  • Working closely and collaboratively with the team’s program coordinator.

Skills and Experience

  • Tertiary qualifications and registration with a relevant professional registration body in a field such as psychology, social work, mental health nursing, or Aboriginal and Torres Strait Islander Health is essential.
  • Experience in consumer assessment and care planning.
  • Extensive experience working with clients with severe and persistent mental health issues.
  • Demonstrated initiative, innovation and ability to contribute to the development of the service.
  • Ability to work effectively with a range of primary care providers across different health sectors.
  • Ability to form and identify strategic partnership.
  • Extensive knowledge around local sector service providers.
  • An understanding of National Disability Insurance Scheme.
  • Excellent telephone manner and general communication (verbal and written) and negotiation and interpersonal skills.
  • Accuracy and attention to detail.
  • Superior organisation and time management.
  • Ability to work independently and as part of a multi-disciplinary team.

The successful candidate is required to have a DCSI (Department of Community and Social Inclusion) Child Related employment screening.
