Life Without Barriers aims to partner with people (including frail aged people, those who have a disability, families and young people in need, in out of home care, who are homeless, who may experience mental illness) to change their lives for the better. An exciting opportunity has become available for a Case Manager on a permanent full time basis to join our dedicated Out of Home Care team based in Rocherlea.

You will be responsible for developing case plans or individual plans by contributing to the effective planning, implementation and regular review of out-of-home care and outreach services for children and young people, to ensure all Life Without Barriers service-users receive appropriate, quality care services in a safe and supportive environment.

While drawing from your tertiary qualifications and experience, you will have a demonstrated understanding of current case plan practice for children and young people, experience with working with children and young people in out of home care, have strong negotiation and mediation skills and a current driver's licence.

To be successful in this role, you will possess the following:

  • Degree in Social Work, Social Science, Psychology or Human Services.
  • Demonstrated understanding of current casework practice for children and young people.
  • Demonstrated experience in working with children and young people in out-of-home care and foster carers.
  • Demonstrated ability to develop quality case plans or individual plans that are consistent with appropriate policy and procedures.
  • Demonstrated capacity to work autonomously in carrying out agreed case plans, or individual plans, for multiple clients simultaneously.
  • Highly developed analytical skills and the ability to apply these skills in case management.
  • High level organisation and time management skills, which includes the ability to reorganise work priorities to meet changing demands.
  • Demonstrated ability to work with individuals, families and/or groups from strength-based culturally appropriate perspective.
  • The ability to function effectively as part of a multi-disciplinary team.
  • Demonstrate alignment to LWB values as outlined in the LWB way.
  • High level communication, influencing, liaison, negotiation and mediation skills.
  • Current Driver Licence.

All successful candidates will be required to undergo probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the position).
