Latrobe Community Health Service is a leading provider of health services across Victoria and we are now expanding our organisation to deliver the Veterans' Home Care Assessment and Coordination Services into Western Australia and we need your service coordination skills to make this happen.

The Veterans’ Home Care Coordinator will build and maintain partnerships with service providers and clients; as well as oversee the delivery of a phone based assessment and care planning service for eligible Veterans located in Western Australia from our office located in Ringwood, Victoria.

To be successful in this position, you will have:

  • A Nursing or Allied Health degree with an AHPRA registration
  • Previous experience in aged care or community services
  • Demonstrated skills in service coordination, budget management and people management

Please note; hours of work will be aligned to the Western Australia time zone.
