Latrobe Community Health Service is a leading provider of health services across Victoria and we are now expanding our organisation to deliver the Veterans' Home Care Assessment and Coordination Services into Western Australia.
We are seeking enthusiastic, qualified and customer focused people to join our dynamic and progressive team.
The role of the Veterans’ Home Care Assessor is will be to provide the delivery of a phone based assessment and care planning service for eligible Veterans located in Western Australia from our office located in Ringwood, Victoria.
You will be responsible in creating and maintaining good working relationships with health care providers and identifying best client outcomes.
