About the role:

As an Aged Care Coordinator you will enhance the quality of life of clients by providing person-centered support and care with a wellness and reablement approach promoting independence and in respect to client’s choice and lifestyle preferences.

This exciting and challenging role will require an individual with experience in Aged Care and community engagement skills to work with the ageing community within the Bayside & Mornington Peninsula in identifying programs to help enrich their life. You will be required to work within the confines of an allocated budget to match individuals to volunteer visiting services whilst providing a positive and meaningful experience.

You will have strong influencing and networking ability and enjoy working autonomously across various networks. Knowledge of the commonwealth home support program & home care packages programs would be highly advantageous.

Duties may include:

  • Respond to referrals indirect and via My Aged Care portal and coordinate bookings into appropriate respite programs
  • Liaise with carers, clients, and/or case managers in order to ensure accurate assessment and care plan development with client goals reflecting a wellness and reablement approach to support and services
  • Regular review and evaluation of services with the client and/or the carer (At least annually)
  • Assist with program development, data entry and statistical or narrative reports as required by the Executive General Manager and Government Departments.
  • Provide leadership and direction to employees, maintaining rosters and actively promoting teamwork

This role is 19 hours per week with the flexibility to choose your own working days and hours!

About you:

To be successful in this role it is a requirement that you have:

  • Appropriate human services qualification and/or experience in community aged services
  • Excellent communication skills both written and verbal, including well developed computer skills
  • Excellent customer service skills
  • Demonstrated ability to be self-motivated and work within a diverse team environment
  • Experience supervising and rostering volunteers / employees / services
