Just Better Care Macarthur / Central West NSW has been at the forefront as a provider of Community Care across South West Sydney for a number of years. Recently we also commenced expanding into the rural areas.

We have grown and developed into a lead provider of HCP, NDIS along with DVA, brokerage and private clients.

We are seeking to employ another full time Case Manager to join our team in Client Services in the Campbelltown Office.

As the Case Manager you will be undertaking the assessments, reviews and care planning of a group of clients.

Along with your one on one client commitments you will be supporting the community care staff who are working with your clients to ensure that a 100% satisfaction result for both clients and care staff is achieved.

To Be Successful you will need to have the following skills and or experiences:

  • Previous experience with assessments, care planning, coordination and follow up to ensure service delivery as per the care plans.
  • Previous experiences working with HCP and /or NDIS in Case Management and / or Support Coordination.
  • Excellent communication, time management and written skills.
  • Experiences abiding by relevant compliance.
