ABOUT US

Established in 1954, IDV is a community base organisation focused on assisting people with a disability within the NE region of Melbourne. The organisation is transitioning towards a community-based service model, working ’one person at a time’ to build individual’s social and economic participation within their community.

Our objective is to collaborate with the individual, their families and other freely given relationships, to deliver sustainable and meaningful engagement for people with disabilities in their communities with a focus on community-based employment.

ABOUT THE ROLE

The Service Coordinator role will model, promote, lead and implement the Mission, Vision, Values of IDV and the objectives of the Strategic Plan. The Service Coordinator will support a portfolio of participants and service delivery hours, spanning across the three life areas in IDV's strategic plan.

You will develop and coordinate the agreed supports and training within individuals' homes, their community and their place of employment, assisting the Service Leader to develop transition pathways to more diverse, individualised and community-based activities for existing participants with the philosophy of one person at a time.

Support in other areas of intake and assessment of new referrals, schedule face to face and all no face-to-face activities.

ABOUT YOU

You will have a a real drive and passion of working with people and helping them achieve their personal goals.

The successful candidate will also need to demonstrate the following key selection criteria.

  • Understanding of the NDIS operating framework and relevant Acts, Regulations, and sector best practice
  • Work under a Person-Centred, strength based approach who is outcome focused
  • Demonstrated competency in ‘place and train’ community-based employment support and skills-based community activities for people with high and ongoing support needs.
  • Understanding of goal and objective setting in relation to identified participant needs
  • Sound understanding of transition programs
  • Experience in Case Management for disadvantaged groups is desirable
  • Exceptional time management and organisational skills
  • Excellent verbal and written communication skills and able to adapt style to suit audience
  • The ability to build relationships with people quickly

  • Current Driver’s License
  • NDIS Worker Screening Clearance
  • Working with Children Check
  • Eligibility to work in Australia
  • Satisfactory Police Check
  • Full COVID-19 Vaccination
