More about the role

The role of the Care Manager (HammondCare At Home) works within the Mission and Mission in Action of HammondCare to ensure that staff have a good understanding of individual client needs and preferences and are able to provide services tailored to the needs of individual clients, delivered within budget and service expectations. The position is responsible for service development, ongoing client assessment, and the development and review of care plans, in consultation with clients and key stakeholders. The Care Manager is also responsible for leading, supporting, and mentoring their team. The Care Manager also works in partnership and consultation with the Clinical Care Manager (Clinical).

The essence of this role:

  • Provide professional and competent day-to-day delivery of services to clients in line with service expectations and HammondCare’s Mission and Mission in Action.
  • Provide professional, capable and aligned leadership for your team in support of HammondCare’s Mission and Mission in Action.
  • Ensure there are sufficient capable and aligned staff in your team to serve clients.

About you

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • Certificate IV in Aged Care (desired).
  • A minimum of 2 years relevant industry experience.
  • Knowledge of the Home Care Packages Program Guidelines, Home Care Common Standards.
  • Experience managing a team of skilled professionals.
  • Experienced in operating effectively in a multi-site environment.
  • A current drivers licence and safe driving record. Willingness to travel between HammondCare’s sites and services.
  • Strong administrative and computer skills.
  • Ability to work independently and as part of a team.
  • Empathy and patience.
