Care Coordinator Nurses can work across our primary healthcare programs including immunisation clinics, Closing the Gap Riverland, Healthy Habits, Integrated Primary Care, and Primary Mental Health Triage.

The current opportunity mainly focuses on Primary Mental Health Triage.

The successful applicant will be a Registered Nurse with experience in and passion for primary healthcare and an interest in developing skills in mental health.

Candidates applying for allied health positions will be APHRA registered.

The successful applicants will be remunerated in accordance with FocusOne Health’s EBA dependent on skills, experience and qualifications. We offer flexible terms and conditions and attractive salary packaging arrangements.

In accordance with the South Australian Children and Young People (Safety) Act 2017, the successful applicants will be required to obtain both a National Police Certificate and a Child Related DCSI screening, prior to employment commencing.
