Fremantle Women's Health Centre (FWHC) is a not for profit, community health organisation dedicated to women-centred care that has been providing health and wellbeing services since 1985. The Community Support and Connector is a pivotal and dynamic position and will be responsible for engaging, supporting and assisting women in a range of ways to meet their needs. It will include co-ordinating and ensuring holistic, integrated and seamless client pathways for women and will be based in Bentley. The role operates within an interdisciplinary team at FWHC and an extremely motivated and dedicated volunteer team at Muslim Women's Support Centre (MWSC).

Key Responsibilities:

  • Provide entry point intake and assessment for women at MWSC and provide care, support, information and client pathways
  • Provide information to and warm referrals for clients
  • Develop person-centred, strengths based case management plans in collaboration with clients that reflect the context of their physical, social and cultural environments, their experiences and their preferences
  • Monitor, adapt and review case management plans with clients and other services providers
  • Work closely with the volunteer team in developing procedures and systems to ensure high level services
  • Consult with other health professionals and community as required
  • Prepare and maintain client notes and ensure data is confidential and securely stored and managed

Skills:

  • Social Work, Mental Health Nurse or other related Human Services Degree
  • Demonstrated significant experience in providing high quality health and human services for individuals and groups, improving outcomes for clients. This includes women and children from diverse cultural backgrounds
  • Knowledge of issues impacting women's wellbeing
  • Strong verbal and written communication skills
  • Ability to work flexibly, autonomously and within an interdisciplinary team
  • Excellent organisation, record and data management skills
  • Ability to use technology and a range of software to communicate with clients
  • Be culturally sensitive and experience working closely with Muslim women and their families
