The Support Coordinator positions will provide Level 2 Coordination of Supports and Level 3 Specialist Support Coordination.

Extended Families has an excellent reputation in quality practice and a supportive team environment. The positions will involve travel to visit participants in various locations. Whilst our primary cohort is people with intellectual, physical, sensory, neurological disability and Autism, we have an increasing number of people with mental illness so experience in mental health services would be strongly welcomed.

  • Strong administration and computer skills
  • Ability to support children and young people with a disability and their families
  • Culturally sensitive
  • Skills and experience in mental health support (preferred)

We are looking for people with qualifications in Social Work, Community Services, Psychology or equivalent.

You will need to obtain an NDIS Worker Screening Check clearance if not already held and where applicable, an international police check is also required. You will be required to provide evidence of your COVID-19 Vaccination status prior to employment.

Ideally the positions will be full time (38 hours), but we are also open to a 3 or 4 day a week position (23-30 hours per week).
