About the role

Eastern Health is seeking a dedicated NDIS Navigator (Nurse) to join our team and help reduce barriers for inpatients requiring NDIS supports for discharge

The interface between health services and the NDIS is complex. Inpatients who require changed or new NDIS supports for discharge frequently experience a protracted stay in hospital as they await NDIS decision-making. Health workers have a key role in reducing these barriers, ensuring better outcomes and more timely discharges for NDIS participants to return to the community.

The NDIS Navigator (Nurse) will provide high-quality care through the autonomous use of expert clinical skills and knowledge of NDIS legislation, processes, services, and assessments. This role involves liaising with teams and services to reduce barriers for NDIS participants who are inpatients. The role provides highly effective communication and professional leadership within the multi-disciplinary team across all Eastern Health sites to support staff and patients in reducing barriers to NDIS access.

About you

To be successful in these roles you will possess:

  • Division 1 Registered Nurse with current AHPRA registration
  • Extensive experience and knowledge of NDIS including legislation, processes, assessments, and services.
  • Advanced clinical decision-making skills, including the ability to prioritise presenting clients
  • Advanced clinical skills in patient risk assessment, care plan development, implementation, and care coordination
  • Ability to work autonomously within a multidisciplinary team environment.
  • A strong commitment to high quality care
  • Experience in a senior clinical role
  • Well-developed communication and interpersonal skills to promote teamwork, and a proven ability to relate effectively to people at all levels of the organisation
  • A current drivers’ licence
  • Proficient in Microsoft Office suite

Vaccination against infectious disease is a mandatory requirement of this role. An offer of employment is conditional on you providing evidence that you are currently vaccinated against COVID-19 with booster & Influenza, prior to commencing employment.
