Role Purpose

The NSW Get Healthy Service is a free telephone-based health coaching service for NSW adults aged 16 years and over. The Service helps individuals create and achieve healthy lifestyle goals through the support and guidance of expert health coaches. Diabetes Australia delivers the NSW Get Healthy Service on behalf of the New South Wales government.

The main objective of this role is to offer health coaching and education to enrolled participants, with a specific focus on reducing risk factors, and ultimately improving health outcomes, particularly for the Aboriginal & Torres Strait Islander people. The position also aims to increase engagement within this community.

Report to the Health Coach Team Leader, you will be part of an allied health team responsible for delivering a state-wide telephone based assessment, identification of intervention requirements based on risk factors of chronic diseases and lifestyle modification to promote optimal self-management of health and wellbeing.

Key Responsibilities

  • Conduct outbound caching calls to participants enrolled in the Get Healthy Service program, especially for Aboriginal and Torres Strait Islander people.
  • Identification of appropriate referrals, responding to referrals in a timely manner and recruiting participants suitable for the Get Healthy Service program.
  • Undertake holistic comprehensive assessments in collaboration with the client using motivational interviewing and self-management approaches.
  • Deliver coaching sessions to each participant over a certain period contributing to an established completion rate.
  • Utilise motivational interviewing techniques to support enrolled participants to identify key behaviour change goals and implement strategies to improve their overall health and wellbeing.
  • Facilitation of client generated goals, that are realistic, time limited and based on addressing health and wellbeing needs.
  • Program scheduling and delivery to a dedicated client load ensuring the ongoing efficiency of the Health Coaching service.
  • Ensure accurate participant and service data collection whilst maintaining the integrity and fidelity of the program within a program database.
  • Ensure compliance with relevant legislation and policies and ensure practices are inclusive and free from bias.
  • Model the principles and values of Diabetes Australia Group and behave and communicate as a fair and ethical professional.

Required Capabilities

  • Aboriginal and Torres Strait Islander identified position in accordance with Equal Opportunity Act (2010). Applicants must demonstrate Aboriginality in addition to addressing the required capabilities.
  • Aboriginal Health Coaches and/or Aboriginal Health Workers and Health Practitioners with relevant health qualifications or experience working with relevant clinical areas.
  • Certificate III or above in Aboriginal and or Torres Strait Islander Primary Health Care or similar.
  • Experience working in relevant clinical areas such as: mental health, diabetes, cancer etc with Aboriginal and Torres Strait Islander population.
  • Experience in delivering telephone-based health coaching interventions or similar health programs directed to individual Aboriginal and Torres Strait Islanders/ diverse populations.
  • APRHA accredited (where required) health professional: Indigenous health or social work degree, Dietitian, Exercise Physiology, Physiology, Psychology, Nursing, Midwifery Occupational Therapist, Social Worker, Rehabilitation counsellor (desirable 3 years).
  • Experience working with clients living with chronic diseases.
  • Experience in the assessment and management of risk factors including smoking cessation, diet modification and physical activity.
  • Ability to facilitate client centred goals and structure interventions for goal attainment based on a comprehensive assessment and using telephone coaching methods.
  • Demonstrated time and work prioritisation skills for the daily management of an allocated client base.
  • Experience in performance-based environments delivering programs with established targets.
  • Intermediate (or above) skills in utilising computer-based programs or software requiring accurate data collection and data integrity.
