Communify’s HART 4000 (Homelessness Assessment and Referral Team, 4000 – Brisbane postcode) is a service funded by the Department of Housing & Public Works as part of the Queensland Government’s Responding to Homelessness Initiative. Working in the Housing First model, the Mission of HART 4000 is Achieving Positive and Sustainable Outcomes for Homeless People.

Communify Queensland is a multi-service organisation that provides a range of programs and activities for families and individuals in the community to enhance their quality of life. Our primary concern is the welfare and wellbeing of socially, physically, intellectually and emotionally disadvantaged and vulnerable people in our community. We currently provide Aged and Disability Services, Day Respite, Childcare, Community Development, Community Education, Emergency Relief, Family and Individual Support, Home Assist Secure, Homelessness Intervention, Mental Health Services, NDIS, Neighbourhood Centres, Social & Multicultural Support and Venue Hire.

Communify is looking for a Coordinator to be responsible for the overall management and development of the HART 4000 service. Responsibilities include:

• Leading a team of highly skilled and passionate Homelessness Intervention Workers
• Providing clients with with a safe and secure pathway out of homelessness in to sustainable housing
• Promote the profile and capabilities of HART 4000 and participate in strategic networks and forums
• Participate in the broader management team of Communify
• Oversee and manage the allocation of brokerage assistance
• Ensure the program is managed in accordance to the service agreement, funding requirements and Communify's policies and procedures

It's a challenging time to manage a team of 7 who are Working From Home during this pandemic crisis, so we are looking for someone with exceptional leadership skills, is able to build rapport with team members quickly and demonstrates extensive experience in a human service environment, particularly delivering services to people with highly complex needs.
