This is a great opportunity to join a small and professional team where the work is challenging and fascinating.
Code Black Psychology is a multidisciplinary team consisting of psychologists, social workers, allied health professionals, and legal practitioners. Through teamwork we provide complex case consultations, diagnostic reviews, care team support and coordination, behaviour support and change, bespoke training and advice on policy and framework development.
Our Care Plan Coordinators provide services to care teams supporting persons with complex needs and persistent risks of harming themselves and others.
Care plan coordinators do not work directly with the complex client, rather, they coordinate the services who provide the direct care to the client.
You will be responsible for supporting care teams to implement MACNI program care plans (under the Complex Needs Act 2009) that have been developed to create a shared understanding of the client’s multiple needs and risk. You may also be required to undertake some specialist support coordination under the NDIS in selected cases.
A client with complex needs may present with one or more of:
You will lead the development of a targeted support response to the client which requires shared problem-solving, decision-making, planning and independent expert input.
The successful applicant will demonstrate familiarity with persons with multiple complex needs through significant relevant previous experience. You thrive on being organised and a great communicator, working with stakeholders, implementing services to improve the wellbeing of clients, monitoring outcomes and reporting progress.
Your interests and experience will help shape the role, particularly where your skill set can enhance the team’s ability to provide clear case formulations, diagnostic clarity, and agile interventions. Professional development is encouraged and supported by the organisation.
This is a full-time position: 0.8 FTE may be considered. We encourage a balance of working remotely and in-person attendance at our office in the Malvern area.
You will be part of a supportive team with a focus on collaboration in case discussions, peer supervision and creative problem solving.
Your interests and experience will help shape the role, particularly where your skill set can enhance the team’s ability to provide clear case formulations, diagnostic clarity, and agile interventions. Professional development is encouraged and supported by the organisation.
An attractive salary package will be offered to the right candidate depending upon qualifications and experience, plus super and leave entitlements.
