About the role

We are currently seeking a Disability Support Leader (Team Leader) to join our Disability Services team on a permanent part-time basis (70hrs/fortnight) in Blackheath. This position plays a key role in the day to day provision of direct care in an NDIS independent accommodation service model in which a mature participant lives in their own home. This position requires you to be able to lead from the front, implement key initiatives and work autonomously.

Some key accountabilities of the role include:

  • Leading, mentoring and coaching a team of Disability Support Workers and ensuring cohesion between a multi-disciplinary service model team
  • Ensuring the maintenance of current service support plans, documentation and encouraging the participant to live an active, independent and rewarding lifestyle
  • Maintaining direct oversight of this service model as the participant has a number of complex needs which will require prompt attention
  • Assist with the recruitment of key staff and provide workable solutions to any unforeseen problems which may arise
  • Supervising, supporting and developing team members to provide services at best practice standard
  • Ensuring effective and efficient administrative procedures and processes to support service delivery, including the upkeep of customer files and folders, monthly reporting and business planning activities
  • Providing direct support to customers who require assistance with tasks of daily living, personal care and medication administration
  • Supporting customers in developing skills covering all aspects of personal development and daily living as defined by goals set in their individual plan (IP)
  • Ensuring that service activities reflect CareWest's values, objectives and priorities

Selection Criteria:

  • Minimum Certificate IV in Disability and/or Welfare or equivalent, and/or 12 months' relevant experience
  • Demonstrated knowledge of the aims and objectives of Supported Accommodation program activities
  • Demonstrated experience with direct customer support work in a Supported Accommodation setting
  • Effective verbal and written communication skills
  • Sound organisational skills and ability to use initiative
  • Demonstrated high level interpersonal skills
  • Demonstrated capacity to work independently and as a team member
  • Flexibility and availability to work across a varying roster providing support 24 hours a day, 7 days a week
  • Hold a current Australian Drivers Licence
  • Current First Aid certificate
  • Experience working with clients with complex medical and behavioural needs who may have a range of physical, sensory and intellectually challenging behaviours is beneficial

You will also be required to:

  • Pass a National Criminal History Check
  • Provide us with a current NSW Working with Children Check
  • Pass a pre-employment medical assessment, including drug and alcohol screening
