The Cerebral Palsy League is a non-profit organisation that provides vital support to children and adults with cerebral palsy and physical disabilities. We work alongside our clients to achieve important life milestones like walking, talking, learning, working and living independently.
We are currently seeking a dedicated Service Facilitator for our Toowoomba Service. The purpose of this role is to provide high quality client case management and effectively supervise, train and support various teams of staff within the service resulting in optimised performance and quality of the service within the region.
As a Service Facilitator, some of your key responsibilities include:
- Providing high quality support to clients in implementing, managing and maintaining their individual accommodation and support plans.
- Developing, implementing and overseeing on comprehensive schedules of appointments and activities with health professionals.
- To ensure clients’ personal care needs are safely, consistently and adequately met (includes the ability to provide personal care support and flexibility for rostered on-call responsibilities).
- To undertake direct supervision, support, training and development of Personal Care Staff.
Recruitment and selection of new team members to ensure household vacancies are filled on a timely basis in line with established recruitment procedures.
- Ensure effective communication and coordination between CPL services, staff and stakeholders to achieve the provision of a cohesive and seamless service package.
To be successful in this role, you will have:
- Experience in the provision of community based services for people with high support needs.
- Availability for periodic after hours emergency support.
- Proven ability to implement and maintain effective and flexible strategies for the provision of accommodation and lifestyle support for clients.
- Demonstrated ability to work effectively as a member of a team, take initiative and to work with limited supervision.
- Proven ability in the areas of recruitment, selection and training of staff, as well as providing direct supervision, including implementing grievance and disciplinary procedures to staff.
