This role is a 12-month contract.

Duties and responsibilities (for full details see Position Description)

  • conduct functional, falls prevention and activities of daily living assessments via telehealth and face to face visits, and make recommendations for at-risk people living with chronic health conditions
  • work collaboratively with the multidisciplinary team and client care providers to support completion of timely and comprehensive assessments and recommendations
  • act as resource for clinical nurses to provide allied health support and direction with client cases
  • identify quality improvement opportunities within the Team Care Coordination Program processes

About you (for full selection criteria see Position Description)

  • Occupational Therapist with minimum of 2 years’ experience and registration with AHPRA
  • experience in working with clients with chronic health conditions in their own home or in community settings
  • experience in the assessment of client needs and the prescription of goods to assist in improving independent living
  • understanding and experience of funding streams and organisations with regards to chronic disease
  • high level of critical analysis, problem solving and decision making in order to apply appropriate interventions in complex environments
  • ability to develop and maintain professional relationships with nursing team, clients, carers and other care providers.

  • It is expected that in this role you will maintain up-to-date vaccinations in accordance with current national/state guidelines
  • You may be required to present evidence of vaccinations on request
