The Bolton Clarke Homeless Persons Program actively seeks to engage and offer a holistic primary health care response to individuals and families who are at risk of or who are experiencing homelessness. The program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services.
The Homeless Person Program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services. The key job responsibilities are to provide a holistic primary health care response to individuals and families who are experiencing homelessness. This is a fixed term part time role for 24 months.
This role is as an 'Assertive Outreach' Nurse working within a 'Social Model of Health', in Partnership with 'Launch Housing, located at 'Bob's Place', Dandenong. The Nurse's role is to provide an initial response including health care, to people who are rough sleeping, in conjunction with a multidisciplinary team. The Nurse will understand the social determinants of health and their application to Nursing within a social model of health. Key to this role is the ability to work autonomously and with external agencies, with flexibility across several complex care scenarios, with an understanding of a social justice framework and the ability to assertively advocate for clients across multiple service systems.
