About the role

The Bolton Clarke Homeless Persons Program actively seeks to engage and offer a holistic primary health care response to individuals and families who are at risk of or who are experiencing homelessness. The program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services.

The Homeless Person Program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services. The key job responsibilities are to provide a holistic primary health care response to individuals and families who are experiencing homelessness. This is a permanent part time role for 24 hours a week.

This position is based at McAuley House and supports women who are homeless or at risk of homelessness by providing medium term accommodation, meals and intensive and individualised case management support. The HPP Community Health Nurse operates a nursing clinic on days of work. The role is based in Footscray. The role will include the following:

What do we need from you?

  • At least 5 years Post Graduate experience
  • AHRPA Registered Nurse Division 1
  • A strong interest in primary health care
  • Capability of exercising a high level of professional autonomy and responsibility in a community setting
  • Ability to form and maintain strong collegial relationships across service organisations
  • Experience or interest in working with complex clients
  • Ability to translate existing skills within a new environment
  • Commitment to improving the health and service outcome for people who are marginalised
  • Current drivers licence in the state employed in.
  • If appointed, a satisfactory Police Check will be required
  • Working with Children Check
  • A pre-employment medical assessment may be required for this role
