Banyule Community Health is seeking to employ an experienced and resourceful Referral and Intake worker to join the Team on a part-time basis. This position provides a centralised and coordinated intake response to new and existing clients requiring information and access to services and programs, including to our mental health stepped care program, known locally as LIFT. The Referral and Intake team is part of a community-based Mental Health Hub, with a focus on the mental health effects of COVID-19 on the Victoria community.

The key responsibilities for this position include:

  • Conducting initial needs identification, risk assessment and follow up for a wide range of services, including for mental health services
  • Providing appropriate information and referral pathways for clients requiring access to services
  • Maintaining effective working relationships and communication processes with other BCH teams and external agencies

The key skills required for this role include:

  • Demonstrated experience and expertise in information provision, initial needs identification, risk assessment and referral in community based settings
  • Sound knowledge of the broad range of issues for which people present in community health settings; and of relevant service systems
  • Highly developed interpersonal and communication skills
  • Experience working within the mental health service is highly desirable

About us

Banyule Community Health has two main sites, one in West Heidelberg and the other at Greensborough. The Client Services & Infrastructure team includes Administration, Service Access, Reception, Health Information Services, Data Management, Information Technology, Cafe and infrastructure. The Team staff provides client pathways both internal and external, initial needs identification including risk, eligibility and priority access to services and groups and Information Technology support.
