Advance Diversity Services (ADS) is a dynamic, community-focused organisation, dedicated to providing crucial support to individuals from Cultural and Linguistically Diverse Backgrounds (CALD) within the South-East Sydney region, notably in the St George and Sutherland areas. ADS pertain an unwavering commitment to assist and advocate for members of these communities, with a special emphasis on recent arrivals, refugees, families, women, and seniors.

Their overarching objective is to empower community members by providing the necessary tools and resources to harness their full potential, thereby enhancing their access to the wealth of social and economic opportunities found in the Australian community.

The team leader will oversee the coordination of the newly created Care Finder program. The program aims to target harder to access groups in the community who are 65 years + and who are unlikely to engage with the system due to barriers such as language, cognitive decline, or a reluctance to deal with the government systems. This will likely include people who have been homeless, experienced DV, or have ongoing mental health or AOD concerns.

In this role, you will:

  • Provide supervision and mentorship to a small team of three case workers (care finders) and support them to provide a high standard of service delivery to clients, providing complex case management.
  • Connect with community stakeholders to build strong networks within the Aged Care space and facilitate opportunities for community outreach events and initiatives.
  • Manage referrals from a variety of sources and triage clients to the appropriate Care Finder
  • Liaise with interagency bodies and maintain regular contact with key stakeholders.
  • Provide regular reporting to the funding body, and regularly monitor compliance.

To have success in this role, you will have an energetic, friendly, and assertive leadership style along with an unwavering commitment to supporting your team to achieve positive outcomes for vulnerable members of the community.

You will also have

  • 1-2 years’ leadership experience in the community services sector.
  • Strong understanding of providing person-centred, trauma-informed care, and a demonstrated ability to provide advice on complex case management to the team.
  • Knowledge in working within the Aged Care sector, including My Aged Care, and community support services available in the local community, or a strong willingness to learn.
  • Experience working with CALD communities and demonstrated cultural competency and understanding.
  • A relevant tertiary qualification in social work, aged care, community services or related field
  • Experience coordinating and facilitating outreach events which advocate for the program and services provided.
  • Excellent communication and relationship building skills.

Requirements for the role

  • A valid driver’s license and vehicle with comprehensive car insurance
  • A recent Police check or willingness to obtain.
  • A valid NDIS Workers Screening Check or willingness to obtain.
