Banyule Community Health is seeking to employ an experienced and resourceful Referral and Intake worker to join the Team on a part-time basis. This position provides a centralised and coordinated intake response to new and existing clients requiring information and access to services and programs, including to our mental health program Head to Health and Stepped Care Program.

Key responsibilities for this position include:

  • Conducting initial needs identification, risk assessment and follow up for a wide range of services, including mental health services
  • Providing appropriate information and referral pathways for clients requiring access to services
  • Maintaining effective working relationships and communication processes with Banyule Community Health teams and external agencies

Key skills required for this role include:

  • Demonstrated experience and expertise in information provision, Initial Needs Identification, risk assessment and referral in community based settings
  • Sound knowledge of the broad range of issues (including mental health) affecting people who present themselves in community health settings
  • Highly developed interpersonal and communication skills
  • Experience working within the mental health service is highly desirable

Banyule Community Health is an independent community health service with its own Board of Directors. It provides a comprehensive range of primary care, dental, welfare and social support services from two main sites, one in the heart of West Heidelberg and the other near the shopping precinct of Greensborough. Referral and Intake workers provide initial needs identification, including risk, eligibility and priority for service assessment. They also provide clients requiring access to services with appropriate information, referral pathways (internal and external), and any follow up that may be needed.
