The Role
Reporting to the Operations Manager, this position plays a critical role in the everyday operations of Affirm Care’s Disability services and programs.
The Service and Workforce Coordinator leads service and coordination development, develop service plans and manages the scheduling of client services, oversee service delivery, monitoring, evaluation and reporting, and alignment with Affirm Care’s Code of Conduct, policies and procedures.
Primary responsibilities
Lead Service & Program Development
- Research evidence-based, culturally secure disability service approaches.
- Explore, identify, and implement strategies for growth of all Affirm Care programs
- Identify service and program strengths plus areas for improvement.
- Co-design service improvements or new services with staff and clients.
- Lead delivery service improvements or new services with staff and clients.
Oversee Disability Service Streams
- Monitor service delivery.
- Keep abreast of disability sector developments.
- Track and monitor client referrals into and out of the service.
Manage Monitoring, Evaluation & Reporting
- Oversee client data and record management.
- Gather and review client feedback and case studies.
- Evaluate outcomes and impact of each service and program.
Leading Service Risk Management
- Identify and manage service risks and opportunities.
- Promote a safe and healthy work environment.
- Respond to critical service incidents.
About You
The Service and Workforce Coordinator is a multitasker with excellent communication, planning and leadership skills as well as an upbeat attitude. The role is responsible for ensuring operational efficiency and compliance in service areas as well as workforce planning.
To be successful in this role you will have experience in service coordination, leading teams as well as scheduling of services to meet client needs. You will have an eye for detail, the ability to lead others effectively and a thorough understanding of risk management. You will be an excellent team player as well as, possess exceptional written and verbal communication skills and have the ability to multi-task in a fast-paced role.
Experience, Skills and Knowledge
The Service and Workforce Coordinator will need to have the following experience, skills and knowledge:
Experience
- Ability to demonstrate sound knowledge and experience in service delivery for people with disabilities and their families
- Ability to manage all rostering aspects for both Participants and employees
- Excellent knowledge of NDIS framework
- Excellent knowledge of the Disability Quality and Safeguard framework
- Excellent written and verbal communication skills
- Experience working with people with a disability in home and community support settings
- Experience supporting staff and clients working with complex behaviour presentation
- Delivering community services and programs
- Working in advocacy, community development and community education roles
- Working with people from multicultural and Aboriginal backgrounds

- High level written and verbal Communication: the ability to communicate clearly and concisely, varying communication style depending upon the audience

Knowledge
- Contemporary knowledge of disability sector

- Extensive experience in the disability field attained through previous appointments, service and/or study with a combination of expertise and competence sufficient to perform the duties required at this level.
- Experience in coordination of disability services
- Significant scheduling experience within the Disability Sector, demonstrating highly effective people skills and knowledge of the SCHADS Service and Workforce Coordinator
