Active Linc Home Based Services is a Support Coordination and Allied Health collective, committed to providing high quality, individualised clinical care and support to NDIS participants across the Perth Metropolitan area. As a registered provider of the National Disability Insurance Scheme (NDIS) our team provide an individualised service working collaboratively with participants, families, external providers, mainstream and community services to maximise function and improve overall quality of life.
We are leading Support Coordination provider in WA, and as the NDIS continues its roll out we are experiencing high demand for Support Coordination, and Specialist Support Coordination services. As such we are thrilled to announce we are seeking dynamic and experienced individuals who are interested in joining our team, in full time and part time roles.
As a Specialist Support Coordinator, you will work in collaboration with NDIS participants and their key stakeholders to identify, connect with and coordinate the delivery of supports required. This can include assisting people to explore their accommodation options, facilitate discharge from hospital, with configuring capacity building supports to maintain or improve functional capacity and so on.
