Active Linc Home Based Services is a Support Coordination and Allied Health collective, committed to providing high quality, individualised clinical care and support to NDIS participants across the Perth Metropolitan area. As a registered provider of the National Disability Insurance Scheme (NDIS) our team provide an individualised service working collaboratively with participants, families, external providers, mainstream and community services to maximise function and improve overall quality of life.

About the role

We are leading Support Coordination provider in WA, and as the NDIS continues its roll out we are experiencing high demand for Support Coordination, and Specialist Support Coordination services. As such we are thrilled to announce we are seeking dynamic and experienced individuals who are interested in joining our team, in full time and part time roles.

As a Specialist Support Coordinator, you will work in collaboration with NDIS participants and their key stakeholders to identify, connect with and coordinate the delivery of supports required. This can include assisting people to explore their accommodation options, facilitate discharge from hospital, with configuring capacity building supports to maintain or improve functional capacity and so on. Your skills, and experience

Successful candidates for this role will have (criteria):

  • Relevant tertiary qualification and/ or experience in coordination, disability or community services
  • Allied and or health experience
  • Awareness or knowledge and understanding of the National Disability Insurance Scheme and the disability sector in WA.
  • The ability to take initiative, and actively assist participants with NDIS planning, implementation, and reporting.
  • Well developed interpersonal and communication skills.
  • The ability to work within a team dynamic and independently, and be self-motivated.
  • Demonstrated ability to manage multiple and competing priorities in a fast paced and stimulating environment.
  • Proficient report writing skills, with the ability to communicate complex issues in ways that other people can understand.
  • A genuine passion for working with people and being a catalyst for establishing and maximising the supports, building capacity and working toward achieving people’s goals
  • Valid Australian Driver’s Licence and reliable vehicle.
  • NDIS Worker Screening
  • Working with Children Check.
