Complex Case Manager / Community Coordinator - Disability, Aged Care, Mental Health

The opportunity:

We are seeking experienced case managers, with an allied health, community services, or mental health background, assist young people at risk of or living in residential aged care (YPIRAC) and their families, transition to more age appropriate accommodation and access to an customised range of services, to meet their whole of life needs.

You will be responsible for end-to-end case management including:

  • Identifying an individual’s goals and undertaking assessments to understand the supports required to transition and assist a young person live as independently as possible.
  • Engaging with Hospitals and Health Services to ensure young persons under the age of 65 years, receive early intervention supports, so they are not discharged into a residential aged care facility.
  • Applying critical reasoning and thinking to ensure all decisions/recommendations appropriately meet the needs of the young person in terms of their goal for community independence and/or engagement.
  • Working collaboratively with services providers, government agencies, and other key stakeholders to coordinate the most effective services and supports to meet the age-appropriate needs of the young person living in residential aged care or independently in the community.
  • Assessing and actively monitoring the effectiveness of selected strategies and supports
  • Maintaining effective and timely reporting of client interactions.
  • Working collaboratively with all teams across the national program, in the design and implementation of internal quality assurance, complaints and continuous improvement systems.
  • Hosting information sessions and providing regular updates to YPIRAC, families and stakeholders about the program.
  • Contributing and supporting the YPIRAC program research through the regular collection of research data and active participation in Community of Practice forums.

About You:

  • Tertiary qualifications in an allied health or other relevant, discipline with at least 5 years (or equivalent) of full time work experience which has equipped you with transferrable skills to provide undertake assessments and complex case management support to YPIRAC.
  • Highly developed understanding of the needs and perspectives of YPIRAC
  • Critical reasoning skills to ensure a high degree of integrity in the decision-making processes.
  • Flexibility and insightfulness and an ability to tailor established service delivery to meet the circumstances of each person
  • Ability to set clear expectations and engage in difficult conversations in a simple and empathetic manner.
  • A current driver’s license, with the ability to travel inclusive of air travel and overnight accommodation to meet the needs of young people in more rural and remote areas or being discharged from hospital.
  • Compliance with Pre-employment checks (including Working Screening Checks)
  • Ability to work a minimum of 3 days per week.
  • Completed vaccinations for COVID-19 and seasonal flu to allow access into Residential Aged Care settings.
