Australian Multicultural Community Services Inc (AMCS) is an established community based organisation with over thirty years of experience in providing support and welfare services to Victoria’s multicultural communities. Summary of Position

In this autonomous role you will be responsible for managing a portfolio of community based clients on Home Care Packages.

You will focus on identifying tailored care solutions to enable our clients to achieve their goals and enhance their quality of life.

You will also oversee clients home care package budgets as well as connecting professionally with family members and external organisations to serve the client’s best interests.

You will be regularly travelling to meet clients in the community and liaising with the internal and external contacts to ensure the smooth delivery of care services.

Key Selection Criteria

• Degree in Social Work, Diploma in Community Services, Case Management or equivalent is essential;
• Relevant experience in the planning, co-ordination and delivery of services to the frail aged and persons with disability in their own homes incl. care management skills, assessment, care planning and coordination and conflict management;
• Knowledge of Italian language and an understanding of the cultural barriers faced by the elderly necessary;
• Demonstrated ability to prioritise, plan and organise a busy workload;
• Demonstrated experience in budget monitoring;
• Experience in supporting clients with dementia;
• High level of motivation and a team orientated approach;
• A high level of professionalism and interpersonal skills;
• Ability to show leadership and initiative;
• Ability to appreciate that all Clients have the right to respect and confidentiality;
• Ability to work in a team environment;
• Advanced communication and negotiation skills;
• Demonstrated decision making and analytical skills;
• A current Victorian driver’s license and police check.

Employment is subject to satisfactory criminal history prior to commencement of employment.

Job Type: Full-time.

Salary: $60,000.00 to $65,000.00 /year.
