Part Time; to cover existing educators leave 1 day per week until 14 July then 2 days per week until 20th September 2023 with the possibility continuing ongoing employment of 2 to 3 days per week thereafter. This position is suitable for either an experienced CDE. New graduate CDE’s are also encouraged to apply.

Diabetes Education Outreach Service is part of the APASS Group and is seeking an accredited and registered Diabetes Educator for our Allied Health Professional team who are involved in primary/clinical care services.

This position is responsible for the delivery of advanced level of Diabetes education, prevention, assessment and management. This position also promotes and maintains collaborative relationships and coordination as required with clients, carers and members of the multidisciplinary health care team.

A’Bell Planning and Support Services (APASS) is a National Disability Insurance Scheme (NDIS) registered provider in Bundaberg, Australia. APASS is a Business to Consumer (B2C) company providing services of Support Coordination, community nursing – Diabetes Nurse Educator, Social Worker and Occupational Therapy with a renewed focus to also include other Allied Health Professionals including Speech Language Pathologists. Essential:

  • Registered Nurse Division 1, with current registration with the Australian Health Practitioner Regulation Agency (AHPRA), or Accredited Practicing Dietitian with current registration with Dietitians Australia.
  • Post Graduate qualifications in diabetes education and Credentialled Diabetes Educator (CDE).
  • Member of the Australian Diabetes Educators Association (ADEA)
  • Excellent theoretical and clinical knowledge on all aspects of diabetes and its management, including client centred Self-Management strategies and an understanding of the clinical and social issues relating to diabetes management.
  • Advanced interpersonal, written and verbal communication skills, and an ability to communicate with a diverse range of people.
  • Ability to work independently without supervision, as well as an effective member of the multidisciplinary team, and be able to demonstrate initiative, enthusiasm and flexibility.
  • Computer literacy, and competent in using a range of information technology applications.

Diabetes Education Outreach Service (DEOS) is a locally owned business run by Pat A’Bell a Credentialled Diabetes Educator and Registered Nurse who has been working in diabetes education for over 30 years. Pat has worked with a variety of cultures and age groups from children to the elderly, which involves educating clients, family, significant others and staff.

Diabetes Education Outreach Service delivers Diabetes Self-Management Education; Individual consultation; Education for Health Professionals; Group consultation; Nursing-home visits and home visits. Covering education for:

  • Newly Diagnosed and currently diagnosed - Type 1 Diabetes; Type 2 Diabetes;
  • Pre-diabetes;
  • Gestational Diabetes;
  • Insulin initiation;
  • Insulin Pump Therapy – Certified Insulin pump Trainer with; Medtronic, Animas and Roche as well as Dexcom and iPro Continuous Glucose monitoring systems.

THE ROLE:

This is a crucial role that will include the organisation of specialist care in accordance with the NDIA guidelines. This position is open to all applicants seeking full time, part time or casual employment.
