About the Role

The position will be critical to providing a health-focused outreach response which engages service users in order to create linkages with The Living Room Primary Health Service and a range of external health, outreach and housing services. The Mental Health Nurse will form part of our multi-disciplinary outreach team to provide high quality, compassionate, person-centred care and collaborate with clients and the team to achieve identified goals and outcomes.

This is a part time role that requires 30.5 hours per week.

What you'll do

  • Provide comprehensive mental health assessments and short-term case management to assist clients who either experiencing or at risk of homelessness.
  • To provide clinical support and expertise to members of the AHOS team, including the Secondary consultations, joint care planning and discharge planning as required.
  • Conduct assertive clinical outreach to engage and assess people sleeping rough in Melbourne’s CBD utilising appropriate nursing interventions.
  • Ensure high-quality care coordination processes for people engaged through the program interventions with a focus on linkages with Living Room services and programs.
  • Triage and assess new clients, conduct clinical assessment and screening.
  • Liaise with Area Mental Health Services, community and allied health services to ensure care planning and appropriate referrals and support to individuals with multiple and complex healthcare needs.

What you'll bring:

  • Current AHPRA registration as Enrolled Nurse with further qualifications in Mental Health, or other relevant qualification.
  • 3+ years’ experience in Mental Health as an Enrolled Nurse
  • Evidence of Continuing Professional Development
  • Experience and/or demonstrated commitment to working with people experiencing homelessness and marginalised populations experiencing psychiatric, medical and substance use conditions within a harm reduction model of care
  • Demonstrated skills and experience in working both autonomously and within a multidisciplinary team
  • Understanding of the issues and implications of homelessness on clients with complex health and social needs
  • Knowledge of current health promotion and harm reduction strategies to reduce the health risks associated with multiple and complex care needs.

Must Have’s

  • Current AHPRA registration
  • Qualifications in Mental Health or other relevant qualifications
  • Valid Passport/Australian Working Rights
  • Police Check
  • Working with Children Check
