About the Role

The After Hours Outreach Service Coordinator is a dynamic new role that forms part of a multidisciplinary team based in Melbourne’s CBD. The After Hours Outreach Service (AHOS) team utilises an assertive outreach model to assist adults with complex health needs who are primarily vulnerable, sleeping rough, living in insecure accommodation or accessing services in the City of Melbourne. The role requires specialist skills and experience in community nursing and servicing people who have complex needs.

This is a part time role that requires 36 hours per week.

What you'll do

  • Lead a multi-disciplinary team of community and mental health nurses and outreach staff to ensure high clinical standards.
  • Conduct assertive clinical outreach to engage and assess people sleeping rough in Melbourne’s CBD utilising appropriate nursing interventions.
  • To provide clinical support and expertise to members of the AHOS team, including the provision of comprehensive nursing assessments, joint care planning and discharge planning.
  • Actively participate in relevant networks and external meetings, by providing clinical leadership and expert knowledge of community resources and processes.
  • Develop and monitor care coordination processes for people engaged through the program interventions with a focus on linkages with Living Room services and programs.
  • Develop practice partnerships with outreach services, emergency departments, homelessness service to improve health and housing outcomes for people sleeping rough in Melbourne’s CBD.
  • Develop a model of integrated care with the existing service system to ensure enhanced service coordination and collaboration.

What you'll bring:

  • Registered Nurse with current AHPRA registration with further qualifications in Community, Mental Health or other relevant qualification/s
  • Demonstrated experience in delivering health outcomes for vulnerable clients
  • Experience working in community health and a strong commitment to social model of health and trauma informed care principles
  • Understanding of the issues and implications of homelessness on clients with complex health and social needs
  • Demonstrated experience in managing and leading a team of professional staff
  • Demonstrated experience in establishing health pathways and coordinated care for clients

Must Haves:

  • Current AHPRA registration
  • Valid Passport/Australian Working Rights
  • Police Check
  • Working with Children Check
