The Regional Assessment Services Home Support Assessor is responsible to undertake face to face Assessments using the mandated National Screening and Assessment Form (NSAF), in the My Aged Care Assessor Portal, create Support Plans and match and refer clients for a Comprehensive Assessment, service delivery or linking service support such as case management, assistance with housing, or non-aged care funded supports.

This position would ideally suit candidates who are seeking a casual role with 2-3 days work each week and extra days when permanent staff are on leave or unwell.

The role reports to our (RAS) Team Leader/Manager. Your Side Regional Assessment Service covers the Regions of Inner West of Sydney and Northern Sydney with positions available in both regions.

About You:

Desirable

  • Possess a Statement of Attainment (SOA2) certification. (mandatory)
  • Have Experience as a My Aged Care Regional Assessment Service (RAS) Home Support Assessor (HSA).

Essential

  • Experience as an aged care case/care manager with assessment experience and skills.
  • Communication and assessment skills which provide empathy and understanding of the client, carers and families and provide a reablement focus to the assessment and support plan created with the client and carer.
  • You will need to demonstrate a thorough knowledge of funded and non-funded Service Providers in the Region in which Assessments are undertaken.
  • Have a sound understanding of the Aged Care system and Community Home Support Program (CHSP).
  • Ensure the protection of all Client Records from loss, damage or misuse.
  • Maintain each Client’s right to privacy, dignity, and confidentiality, particularly during the collection, use and disclosure of personal information.
  • Have you own reliable vehicle with comprehensive insurance.
