We are looking for an Out of Hospital Care (OHC) Program Manager to have overall responsibility for the OHC Program at Your Side (this includes ComPacks, SASH, End of Life packages). OHC packages are non-clinical packages of care.

Your key relationships will be with the NSW Ministry of Health, the Murrumbidgee Local Health District (LHD), and our OHC partner, Community Services #1. This will include reporting to the Ministry of Health on the quality of the OHC program at Your Side, and meeting Ministry of Health and LHD set KPIs. You will also need to monitor and manage the service quality and KPI compliance of our Murrumbidgee based subcontractor. In addition, a proportion of your role will include performing intake and assessment of OHC eligible clients, and managing the Intake Coordinator who will report directly to you.

This is a new permanent role reporting directly to our Customer Care Director, and based within our Chatswood office with the opportunity for flexible work arrangements including the potential for part-time or full-time hours, and working from home.

About You:

  • Previous work experience in quality/compliance, preferably within public health and/or community services
  • ComPacks, SASH and End of Life packages experience crucial
  • Previous work experience in intake/assessment/referral
  • Experience in policy writing and review
  • Able to establish a new program including the set up of policies/procedures
  • Strong focus on a continuous quality improvement
  • Excellent presentation skills
  • Customer service orientation
  • Good relationship building skills with local health district health professionals, and administrators
  • Exceptional administrative skills and attention to detail
  • Comfortable and proficient using an online client management system
  • Ability to travel several times per year to the Murrumbidgee region
