Are you a driven and dynamic individual who is passionate about supporting people with a disability? Join Yooralla in an exciting, high paced role as a Support Coordinator, responsible for assisting a participant to understand and implement their NDIS plan.

To be successful in this role, you will bring:

  • An in - depth knowledge of the disability sector
  • A keen understanding of the NDIS key principles
  • Highly developed communication and interpersonal skills, both written and verbal
  • Relevant experience working within the Human Services, Disability or allied health area
  • Effective time management skills
  • Ability to build strong working relationships and establish ongoing rapport
  • Experience within a similar position is preferred
  • Ability to work in a fast-paced setting and manage workload with competing priorities
  • Independence and proactive learning style and sound problem-solving skills

About the Role

The Support Coordinator position can be based in either Wangaratta or Benalla.

The role is focused on strengthening a participant's ability to navigate the NDIS and to empower participants to access supports to enable each person to participate more fully in their community of choice. The Support Coordinator plays an integral role in the development and achievement of a participant's goals to maximise opportunity and independence.

The role involves;

  • Utilising high quality engagement strategies to strengthen a participant's ability to navigate the NDIS
  • Linking participants to identified services to maximise opportunity and independence
  • Supporting the participant to evaluate goals and provide outcome reports to the NDIA
  • Ensuring high quality record keeping and reporting maintained

Requirements

To be considered for this role you must attach the below mandatory requirements with your application:

  • Ability to obtain a NDIS Worker Screening Check clearance prior to your commencement
  • Current valid Australian driver’s licence
  • Evidence of eligibility to work within Australia (Australian Passport/Australian Birth Certificate/ Australian Citizenship Certificate or International Passport & Visa)
  • A tertiary qualification in a relevant discipline appropriate to disability, allied health, social science or welfare practice
  • If you have lived or worked overseas in the last ten years for a period of 12 months or more, we require a certified copy of your International Police Check, or evidence of applying for an international police check. The Police Check must cover the entire period you were overseas.
  • A copy of your Working with Children Check (WWCC) (Employer)
