Wurli-Wurlinjang Health Service is an Aboriginal Community Controlled Organisation delivering primary and clinical health care services to the Aboriginal people in Katherine and surrounding communities. Wurli prides itself on delivering a range of health and wellbeing services to the community in a style and manner that makes clients feel welcome and comfortable. Services include acute and general care, chronic disease, Women’s & Children health, men’s health, alcohol and other drugs, social and emotional wellbeing and client support services.

The Case Manager is a pivotal role in the delivery of health and wellbeing services in a range of programs at Wurli and provides a professional linkage between Wurli, individuals, families and communities to establish and maintain positive relationships. The Case Manager is predominantly responsible for the delivery of client assessment and the development and ongoing monitoring of client case/care plans. In addition to this, the Case Manager must ensure that the program provides holistic, person-centred and culturally appropriate case management, education advocacy, and therapeutic services for Aboriginal and Torres Strait Islander individuals, families, and groups according to the program-specific funding requirements agreement.

An example of the duties to be performed include:

  • Initial client assessment in accordance with the appropriate Risk Assessment Framework tool
  • Development of case/care plan in accordance with client needs
  • Action/record appropriate referrals in accordance with case/care plans to other team members and/or services in or external to Wurli
  • Review of client progress and modification of case/care plans as required
  • Provide education in accordance with the case/care plan and in conjunction with clients’ needs as they present

Applicants must be able to demonstrate:

  • Certificate IV in Community Services / Certificate IV in AOD / Mental Health other relevant fields
  • Minimum of two years experience in support or case worker roles that incorporates client liaison, education, advocacy and support
  • Exposure and experience in support services in a particular field such as AOD, Social and Emotional Wellbeing, Family and Domestic Violence, Mental Health etc.
