ABOUT THE ROLE

The MYAP program is a specialist youth case management support program for young people in the inner west region of Melbourne who are homeless or at risk of homelessness. The program has a focus on culturally and linguistically diverse young people.

The Early Intervention Case Manager is an exciting new role within the MYAP team that we have developed to provide early intervention support to young people who are at risk of homelessness, to divert them from the homelessness system and/or help direct them to the support they need. The role also entails developing networks and relationships that will enhance MYAP’s response to young people in need.

With the support of the Team Leader, General Manager and program team, the Early Intervention Case Manager will:

  • Respond to and manage all new youth enquiries at Wombat
  • Provide Housing and sector information to clients as required
  • Complete a supported referral (IAP) for those who are experiencing homeless or at risk of homelessness
  • Develop community relationships, pathways and partnerships with local schools, health services and real estate agents
  • Offer secondary consultation to external care teams or workers
  • Provide short term case management support to clients as required

Work Arrangements

  • An ongoing position
  • .8EFT - 4 days per week
  • A competitive salary at SCHADS Award Level 5
