About the opportunity

This role is a key leadership position within the Aged Home Care Package team and contributes to the development of evidence-based, high quality and innovative clinical services. The position provides oversight to the coordination and delivery of case management services, leads a multi-disciplinary team and is primarily responsible for providing clinical leadership and direction to ensure service delivery is responsive to consumer needs. About you

To be successful in this role you will need a bachelor level degree in an allied health discipline such as social work, psychology, nursing, occupational therapy or a related discipline and full registration, or eligibility for full registration, with the relevant professional body.  Our ideal candidate will have advanced clinical skills and demonstrated experience in a range of aged care services, knowledge of assessment and allocation of referrals and a background in service development.
