We have outstanding opportunities for passionate and experienced people to work with a highly reputable and iconic community services organization. The Home Care Package Case Manager works in partnership with consumers, carers, families and informal supporters to facilitate the consumer-directed support and services that may be required to meet the client’s goals; assistance to help older people remain living in their home in line with Uniting principles and/or funding guidelines.
We're looking for motivated people who have a genuine interest in and commitment to working with vulnerable people in our community. You will have a diploma qualification in Social Work, Community Services or equivalent and experience working in an aged care setting. Empathy, resilience, outstanding personal qualities and a preference for working within a collaborative team environment are qualities that will succeed in the role.
