About the opportunity

We have outstanding opportunities for passionate and experienced people to work with a highly reputable and iconic community services organization. The Home Care Package Case Manager works in partnership with consumers, carers, families and informal supporters to facilitate the consumer directed support and services that may be required to meet the client’s goals; assistance to help older people remain living in their home in line with Uniting principles and/or funding guidelines.

About you

We're looking for motivated people who have a genuine interest in and commitment to working with vulnerable people in our community. You will have a diploma qualification in Social Work, Community Services or equivalent and experience working in a aged care setting. Empathy, resilience, outstanding personal qualities and a preference for working within a collaborative team environment are qualities that will succeed in the role.
