The Finding Solutions Plus Program provides short term support to young people aged 12 to 17 years and their families who are referred by Child Protection. The program aims to strengthen the family environment and reduce the need for out of home care.
The Case Worker will deliver creative outreach responses to young people and their families who are experiencing complex and sensitive issues. The role will undertake risk assessments, family mediation and other supports to resolve issues that may jeopardise the young person's placement in the family.
