About the opportunity

We have an opportunity for a practitioner with experience in social work, psychology or disability fields to join our Gippsland Child, Youth and Family Services Program. The Case Worker will provide intensive support to families of children who present with highly complex behaviours due to disability and are at risk of requiring care outside of the family home. The Case Worker will also work with families where the child resides outside of the family home and return to the home is the case plan objective.

Responsibilities of the Case Worker include:

  • Undertaking therapeutic assessments of children & families underpinned by a range of theoretical approaches
  • Developing action plans for children & their families where complex disability support needs are identified
  • Developing parenting strategies that will support strengthening and stabilising of the family
  • Supporting families to navigate the NDIS
  • Advocating for families to engage in identified disability services

About you

To be successful in your application you will meet the person specification in the position description, including having a tertiary qualification in social work, psychology or a related discipline.

You will also have:

  • Previous experience working directly with children and young people with highly complex behaviours due to disability
  • Demonstrated case management experience within the child, youth and family welfare or disability sectors
  • Well-developed risk assessment and risk management skills
  • A trauma informed and family strengthening case practice approach
  • Excellent communication, organisation and time management skills
