We have an exciting opportunity for a passionate Care Manager to join our Carer Support Program. The program supports care relationships between carers and consumers by providing quality respite care to clients aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people, so that regular carers can take a break from their caring responsibilities. Respite care services are provided through the Commonwealth Home Support Programme (CHSP) and the Care Relationships and Carer Support Sub-Programme (CRCS). The programmes deliver planned flexible in-home and out of home centre based respite care services.
As a Care Manager, you will work closely with consumers and carers to develop, monitor and review person-centred goals and care plans tailored to the individual needs and preferences of each client. The Care Manager will ensure that care plans reflect the principles of wellness, reablement and consumer choice and actively support carers and consumers to achieve their care plans goals.
