In this role, you will be directly responsible for case management and program support of young people who are disengaged from mainstream education including Aboriginal and Torres Strait Islander young people, participating in Whitelion Case Management Services.

About the Role:

This role is based in Mount Gambier in the South East of South Australia and will be case managing young people referred from schools to Whitelion within our Assertive Outreach Case Management program. We are seeking committed, highly motivated and passionate case manager who has experience working with highly-complex, disadvantaged young people. It is essential that applicants hold minimum of a Certificate IV in Youth Work or equivalent and a current DCSI clearance.

Responsibilities include:

• One-to-one case management, supporting at risk young people with complex needs to achieve their goals
• Developing case plans, maintaining client records and deliver meaningful support
• Ensuring that services delivered are effective, efficient and client focused
• Working effectively in partnership and collaboration with local community groups
• Building and maintaining relationships with internal and external stakeholders
• Providing assistance as identified including general independent living workshops and wellbeing group sessions.
