Position Context

WeSupportU is seeking a highly motivated person to work intensively in our Forensic Outreach team. The Intensive Therapeutic/Forensic Support program provides intensive support to NDIS participants who have a history of complex needs, and /or incarceration, to assist them in achieving their goals set out in their NDIS plan. As a member of the multi-disciplinary team, you will be required to work intensively with participants and within a care team including all relevant stakeholders.

Position Requirements:

  • A person/family centred focus
  • Genuine interest and passion for working with people with a disability
  • Working with participants to achieve their goals in line with their NDIS plan
  • Ability to manage a caseload and complete timely notes and other admin tasks
  • A commitment to occupational health and safety (OHS)
  • Willingness to travel within Melbourne
  • Working with Children Check and Police Check
  • Maintain confidentiality of participants
  • Own vehicle and current driver’s license
  • Willing to undertake the training relevant to the position.

Position Duties:

  • Engaging and building rapport with participants who may have a range of complex issues including risk
  • Ability to develop tasks to support participant goals in line with service framework
  • Engage with families and other services to collaborate on implementation, and evaluation of interventions
  • Provide key summary reports to NDIS and a range of organisations/ professionals
  • To engage in quality assurance processes as part of a continuous improvement model
  • Participate in care team meetings and professional team meeting to support the development of interventions that successfully impact NDIS goals

Key Selection Criteria:

  • Experience in welfare/ disability and or mental health
  • Comprehensive understanding of best practice engagement interventions for participants with complex special needs.
