Due to significant growth are currently seeking a Full time Intake and Referral Officer to join our Adult Services Team.

Reporting directly to the General Manager, Adult Services , the primary objective of the Intake Officer is to provide advice and support to potential clients and their families to ensure a positive onboarding experience and accurately complete referral/intake records.

You will be the first point of contact for potential participants. You must have a strong understanding of the National Disability Insurance Scheme and be able to provide potential participants and their families accurate and clear advice about our disability services.

You should prepare a succinct application demonstrating why you believe you are the best person for the position. Key Responsibilities

  • Respond efficiently and appropriately to all prospective participant enquiries via email and phone

  • Collaborate with participants and their families to understand their needs and preferences

  • Collaborate with the service delivery team to ensure service suitability to meet specific care needs

  • In collaboration with the Business Facilitation team ensure service agreements that accurately developed to reflect client requirements

  • Follow all principles of the National Disability Insurance Scheme and relevant legislation

  • Provide the relevant information about our service capacity and limitations to clients

  • Provide reports that include new client data, exits, trends and positive outcomes

  • Work with funding bodies

  • Liaise with internal and external stakeholders

Skills & Experience

  • Certificate IV in Disability, Mental Health or Community Services or the equivalent demonstrated knowledge and/or relevant working experience

  • Strong communication skills with the ability to build and maintain positive relationships

  • Sound understanding of person-centred thinking, the principles of the NDIS and the National Disability Service Standards

  • Demonstrated experience in managing competing priorities and to deliver outcomes within agreed timeframes and quality standards

  • The confidence to maintain service user privacy and confidentiality

  • Excellent organisational skills, problem solving ability, adaptability, and the ability to work in a busy high-volume environment

  • Attention to detail

  • Ability to take initiative and work autonomously

  • Drivers licence valid in Australia

  • Completion of the 'NDIS Worker Orientation Module" www.https://training.ndiscommission.gov.au/

  • Completion of Joint Protocol module https://www.acwa.asn.au/joint-protocol-training-for-itc-and-residential-care-workers/

Holding a current Working with Children Check and completing a National Criminal History Record Check clearance are requirements of this position.
