About the role

In response to COVID-19 and further support for our community, this position is a key role in the provision of care needs assessment and care plans to community members who contract coronavirus. The Covid positive care model ensures covid positive patients receive safe and quality care in their home setting and have access to higher levels of care as required.

The Lead - Remote Health Pathways position provides clinical leadership within the interdisciplinary health care team. This position is responsible for contacting overseeing the Care Pathways team in supporting Covid positive community members to seek consent to assess and direct them into the most appropriate clinical pathway with social supports established.

The Care pathways team operates 7 days per week with shift hours varying depending on service need, incumbents will be required to participate in shifts as rostered

Key duties:

  • Coordinate the setting up and management of Clinical Referral Pathways team
  • Manage a team of team leaders and care navigators
  • Manage integrity of data collation and maturity of reporting to key stakeholders
  • Liaise with key stakeholders in region to coordinate response in the spirit of partnership and shared client view
  • Practice collaboratively and provide clinical leadership within the interdisciplinary health care team to promote the safety, security and personal integrity of individuals and groups
  • Provide clinical and operational leadership for professional and clinical practice, education and research in relation to infection control and prevention

About you:

  • Senior Nursing Management and Coordination Experience
  • Up to date infection prevention and control practice and experience
  • Excellent stakeholder management skills
  • Excellent leadership and management skills and experience
  • Experience of managing a team of staff
