About the role

cohealth is seeking a Dual Diagnosis Clinician to join the Homelessness to a Home Program. In partnership with Unison and Melbourne City Mission and other agencies, the Dual Diagnosis Clinician will use multi-disciplinary, assertive, integrated and flexible approaches which addresses immediate crisis support needs, through to longer term support to establish safe and affordable long-term housing with Mental Health, AOD and other health supports in place.

Your daily duties will vary but may include:

  • Provide direct service delivery to clients with a complex dual diagnosis, either individually or in group settings. Support may include brief interventions, case management/care coordination practices
  • Provide high quality, recovery-oriented, evidence-based primary care services to clients, and groups of clients, with a dual diagnosis
  • In conjunction with H2H team, provide crisis intervention management for complex and challenging clients
  • Work flexibly to meet peoples varying needs, including on an outreach basis
  • Provide secondary consultations to other cohealth staff as required and assist in developing strategies to effectively manage clients with complex needs

We are looking for someone with:

  • Tertiary qualifications in Nursing / Psychiatric Nursing / Social work / Psychology and eligibility to join the relevant professional body (including AHPRA)
  • At least three years of mental health and/or AOD sector experience
  • Relevant professional and/or lived experience and a sound understanding of the key issues affecting people experiencing homelessness/harm reduction principles
  • Highly developed communication, interpersonal and negotiation skills
  • Ability to work within a multi-disciplinary team, as well as independently
  • A good understanding of Community Health, its principles and organisational forms
  • Ability to speak a relevant community language would be an advantage
