About the Role

This position is within the HARP Complex Care - Service Facilitation Team. The Service Facilitator is responsible for providing comprehensive assessment and targeted interventions for persons who frequently present to RMH with a range of complex psychosocial needs, such as Homelessness, Substance Abuse, Mental Health, ABI, Cognitive Impairment, Frailty, Social Isolation and/or other complex health issues. The position co-ordinates safe transition and continuity of care, by conducting comprehensive assessments, coordinating specialist clinical and community services to ensure sustainable services are able to support clients in the community. The Service Facilitator will work collaboratively with the Service Facilitation team and broader HARP program to ensure client outcomes are coordinated and optimised and will contribute to quality service developments.

Your daily duties will be diverse however, will include:

  • Conduct and document comprehensive assessments that accurately identify risks and issues which may result in clients representing to hospital
  • Develop, implement & monitor care plans which address identified issues in a timely and appropriate manner in collaboration with clients, carers, their local medical teams and program partners
  • Develop and maintain excellent working relationships with clients, carers, HARP colleagues, cohealth, Merri Health, Bolton Clarke, RMH, GP's, local government, other acute, sub-acute & community services
  • Regularly review team and program operations; plan, implement and evaluate evidence-based improvements as required in collaboration with HARP colleagues & Team Leader
  • Maintain client records (electronic and written) as per Health Records Victoria Act 2001, Health Independence Program Guidelines, TCM guidelines & organisational policy and procedure. (Refer to Appendix one for further details)
  • Proactively identify own learning needs as outlined in individual work plan, and seek appropriate training to address these needs

About You

Our ideal candidate for this candidate will have:

  • Demonstrated experience in comprehensive care needs assessment and care planning
  • Experience in outreach service provision and conducting risk assessments with complex care clients
  • Ability to manage crisis situations in order to maximise the safety of clients and self
  • Ability and willingness to engage with clients with multiple high-risk complex needs and challenging behaviours
  • Demonstrated ability to work within an interdisciplinary team
  • Is a Registered Nurse, Social Work or Allied Health
  • Excellent assessment and clinical reasoning skills
