About the role

HRAR - High Risk Accommodation Response for COVID is a newly funded program designed to support accommodation providers to prevent COVID 19. The program is an end to end program that works with each accommodation area to assess readiness, support readiness and in the event of an outbreak manage the situation.

This position sits within the COVID Response Team and is a key role in the provision of assessing and supporting community members who contract coronavirus. The Team Leader will supervise Care Connectors who notify people of their COVID positive status, advise of self-isolation guidelines and refer people into appropriate health pathways. The Team Leader will conduct case management of especially complex clients.

Key accountabilities include:

  • Conduct in-depth health and welfare assessments
  • Build and maintain strong team working amongst Care Connectors
  • Manage the performance of Care Connectors and or Care Navigators
  • Manage the rosters of Care Connectors and or Care Navigators
  • Work with other Team Leaders and the Remote Health Pathway Lead to develop and improve working processes
  • Liaise with Royal Melbourne Health to provide hospital-based or Hospital in the Home care depending on health status of clients
  • Liaise with Department of Health and Human Services as required regarding the status and care of clients who are COVID positive
  • Connect COVID positive clients to council and independent agencies as required, such as support services relating to food, housing, family violence, alcohol and other drugs
  • Use telephone interpretation when appropriate
  • Ensures precise and timely documentation of client information and data collection
  • Carry out duties in a manner that does not adversely affect their own health and safety of others by reporting all incidents and injuries as well as cooperating with any measures introduced in the workplace to improve Work Health & Safety (WHS)
  • Comply and adhere to all cohealth policies and procedures including code of conduct and values; Perform all other duties as directed, within the limits of skills, competence and training to maximise flexibility and effectiveness

About you:

  • Registered clinician in Nursing or Allied Health
  • Experience managing relationships with other agencies
  • Experience managing staff
  • Experience of planning and organising workloads
  • Ability to use client management databases
  • Stakeholder management skills
  • Flexible and understanding each day can be very different
