About the role

cohealth's Family Recovery Program is newly created and has been established to support culturally diverse families. This support is for employment, education, mental-health recovery and health promotion after a positive COVID-19 diagnosis or a public health order to self-isolate or quarantine. The Case Manager will support improved access, connection to and navigation of, mainstream and other culturally specific support services related to education and training, employment, health and mental health and COVID-19 health promotion.

Your daily duties will vary but will focus on:

  • Undertake a culturally sensitive comprehensive needs assessment with referred families to identify support needs and goals
  • Ensure a trauma informed and cultural safe approach to undertake assessment and development of a recovery plan
  • Act as the main point of contact for the family and provide supports to coordinate their care
  • Actively engage with the family: using persistence, compassion, flexibility, and an understanding of trauma, to build trust and maintain a relationship

About you

  • Demonstrated experience working in community, settlement or public health settings
  • Experience in working with consumers from CALD or disadvantaged communities
  • Demonstrated success with delivering service which is trauma informed and culturally appropriate
  • High level communication and interpersonal skills and ability to work effectively with managers, staff and stakeholders in a state-wide context
  • Experience in delivering health/community services using a Human Rights approach
